About the role
Optimum Care Group is seeking a dedicated NDIS Support Coordinator to join our team in Cranbourne, VIC. This is a full-time position that plays a vital role in connecting participants with the National Disability Insurance Scheme (NDIS) to the services and support they need. As an NDIS Support Coordinator, you will be the key liaison between participants, their families, and service providers, ensuring that individualised plans are effectively implemented and participants achieve their goals. This role is essential to our mission of delivering person-centred care and support that empowers individuals to live their best lives.
What you'll be doing
Coordinating and arranging individualised support services for NDIS participants based on their approved plans and goals
Building strong relationships with participants and their families to understand their needs, aspirations and preferences
Liaising with service providers and community organisations to facilitate access to appropriate supports and services
Monitoring participant progress and ensuring service delivery aligns with plan objectives
Providing regular reporting and feedback to participants and their families about service coordination activities
Identifying gaps in service provision and advocating for appropriate supports and resources
Maintaining accurate and comprehensive records of participant details, service arrangements and outcomes
Supporting participants to understand their NDIS plans and how to best utilise their funding
Collaborating with multidisciplinary teams to ensure holistic and coordinated care
Contributing to continuous improvement of support coordination practices and service quality
What we're looking for
Knowledge of the National Disability Insurance Scheme (NDIS) and how it operates
Demonstrated experience in support coordination, case management, or disability support services
Strong interpersonal and communication skills with the ability to work effectively with diverse populations
Excellent organisational and administrative abilities with attention to detail
Proficiency in computer systems and data management, including familiarity with NDIS planning tools
A person-centred approach to service delivery with genuine commitment to supporting people with disability
Ability to work independently and as part of a collaborative team
Current police check, working with children check and disability worker exclusion list clearance
Current driver's licence and reliable transport (desirable)
First Aid certification or willingness to obtain it (desirable)
