The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling, NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

At the heart of this role is walking side-by-side with Aboriginal and Torres Strait Islander community members as they explore the NDIS access pathway while supporting them to understand their options, build confidence, and achieve meaningful outcomes.
The South West Aboriginal Medical Service (SWAMS) is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. This position is designated under Section 50 of the Equal Opportunity Act 1984 (WA), and as such, only Aboriginal people are eligible to apply.
As a Community Connector – Access for the Ngala Kaaditj Community Impact Team, you’ll play an important role in helping Aboriginal and Torres Strait Islander people understand and access the National Disability Insurance Scheme (NDIS). You’ll be a trusted guide which includes raising awareness, providing culturally safe information, and empowering individuals, families, and carers to explore supports that improve quality of life.
In this role, no two days are the same. You’ll connect with community, collaborate closely with local organisations and service providers, and support individuals through every step of the NDIS application journey. Your work will help people feel supported, informed, and confident navigating a sometimes complex system.
If you’re community‑minded, culturally aware, and passionate about creating stronger pathways to disability supports—especially in remote areas—this role gives you the chance to make a meaningful impact every day.
Please note this role does require some travel to attend and coordinate events. This travel may include overnight stays in regional towns.
