Join us as a Community Aged Care Service Partner and be part of something bigger.
What you’ll doAs a Service Partner in Community Aged Care, you’ll support older South Australians to live safely, independently and confidently in their own homes.
Working in partnership with customers, families, carers and service providers, you’ll coordinate and monitor personalised support plans that reflect each customer’s goals, preferences and available funding. You’ll be a trusted point of contact, helping customers navigate the aged care system, make informed choices and access the services, equipment and supports they need.
