About the role
Miracle Health Services is a trusted provider of community based aged care services across Victoria. We are committed to delivering high-quality, person centred care that enables older people to live safely, independently and confidently in their own homes.
We are seeking a compassionate and experienced Care Partner to join our Support at Home team, based at our Craigieburn office. The position will primarily support clients across Melbourne’s northern suburbs and will involve occasional travel to our regional office in Wangaratta.
About the RoleAs a Care Partner, you will manage a caseload of clients and work closely with them, their families and representatives to understand their goals, preferences and support needs.
You will coordinate personalised services that promote independence, wellbeing, dignity and choice. You will also ensure care is delivered safely and in accordance with Support at Home funding requirements, the Aged Care Quality Standards and Miracle Health Services’ policies and procedures.
Key ResponsibilitiesConduct regular home visits to assess and support clients.
Complete initial and ongoing assessments to identify each client’s needs, strengths, risks and goals.
Develop, implement, monitor and review individualised, person-centred care plans.
Coordinate services and supports in accordance with each client’s approved funding, care plan and preferences.
Work closely with the rostering and service delivery teams to ensure services are arranged and delivered effectively.
Build trusted relationships with clients, their families, representatives and informal supports.
Liaise with general practitioners, hospitals, healthcare providers, allied health professionals, social workers and community organisations.
Monitor client budgets and expenditure to ensure services remain appropriate, sustainable and compliant with funding requirements.
Identify and respond promptly to changes in a client’s health, wellbeing, circumstances or support needs.
Facilitate referrals, case discussions, care team meetings and case conferences when required.
Respond appropriately to incidents, complaints, feedback, risks and safeguarding concerns.
Maintain accurate and timely records of assessments, care plans, reviews, services, communications and incidents.
Support clients to understand their rights, available services, funding arrangements and applicable client contributions.
Contribute to team meetings, quality improvement activities and continuous service development.
Occasionally travel to the Wangaratta regional office to support regional operations, service development and collaboration with local stakeholders.
You are a compassionate, organised and solutions-focused professional who is committed to helping older people maintain their independence and quality of life. You communicate confidently, respond calmly to challenges and work effectively with clients, families, service providers and multidisciplinary teams.
You will be well suited to this position if you have:
Previous experience as a Care Partner, Care Advisor, Case Manager, Care Coordinator or in a similar community aged care role.
Experience supporting aged care clients with complex health, social or care needs.
A qualification or professional background in social work, nursing, allied health, community services, case management or a related discipline, which is highly desirable.
Strong knowledge of the Support at Home program, person-centred care and relevant aged care legislation and standards.
Experience completing assessments, developing care plans, coordinating services and conducting client reviews.
The ability to monitor client budgets and coordinate appropriate services within available funding.
Sound knowledge of risk management, safeguarding, incident management and work health and safety.
Excellent written communication, documentation and record-keeping skills.
Strong interpersonal skills and the ability to build positive relationships with clients, families and service providers.
The ability to work independently while contributing positively to a collaborative team.
Good computer skills and the ability to confidently use client and care management systems.
A genuine commitment to Miracle Health Services’ values of care, respect and excellence.
The flexibility and willingness to travel occasionally to Wangaratta.
A current National Police Check or the ability to obtain one.
A current Working with Children Check or the willingness to obtain one.
Current First Aid and CPR certification or the willingness to obtain them.
Up to date COVID-19 and influenza vaccinations in accordance with applicable requirements.
Australian working rights.
A current Victorian driver’s licence.
Access to a reliable, registered and insured vehicle.
The ability to undertake home visits throughout Melbourne’s northern suburbs and occasional travel to Wangaratta.
