We’re looking for an experienced Clinical Nurse who is passionate about disability care and wants to be part of an organisation that is raising the standard of support within the NDIS sector.

This is more than a nursing role. You’ll be the clinical expert within our organisation, working alongside participants, families, support workers and allied health professionals to ensure participants receive exceptional, evidence-based care.

About the Role

As our Clinical Nurse, you’ll provide clinical leadership across a diverse range of participants with varying disabilities and health complexities.

You will be responsible for ensuring participants receive safe, person-centred care while supporting our support workers with education, guidance and clinical oversight.

Your responsibilities will include:

• Conduct comprehensive nursing and health assessments.
• Develop, implement and review nursing care plans.
• Complete continence assessments and provide evidence-based recommendations.
• Oversee medication management and delegated health supports.
• Monitor participant health and identify changes requiring intervention.
• Support participants with chronic health conditions, complex disabilities and high-intensity supports.
• Develop clinical protocols, procedures and best-practice resources.
• Mentor and educate disability support workers in safe clinical practice.
• Liaise with GPs, specialists, hospitals, pharmacists and allied health providers.
• Assist with hospital discharge planning and continuity of care.
• Maintain accurate electronic clinical documentation.
• Participate in quality improvement activities, audits and incident reviews.
• Support participants through the NDIS by providing clinical reports, assessments and recommendations where appropriate.
• Ensure compliance with AHPRA requirements, NDIS Practice Standards and organisational policies.

About You

We’re looking for someone who enjoys working autonomously while also being part of a supportive multidisciplinary team.

You genuinely enjoy building relationships with participants and empowering others through education and leadership.

Essential Requirements

• Current AHPRA registration as a Registered Nurse.
• Bachelor of Nursing.
• Minimum 5 years’ post-registration clinical experience.
• Experience working within disability, community nursing, aged care or complex community care.
• Current National Police Check.
• NDIS Worker Screening Clearance (or willingness to obtain).
• Current Driver’s License and reliable vehicle.
• Current First Aid and CPR certification.
• Excellent communication and documentation skills.
• Strong clinical assessment and critical thinking skills.
• High level of organisation and ability to work independently.
