The standard caseload will be made up of Orthopedic, Cardiac, Neurology, Pulmonary, and general reconditioning patients in both inpatients and outpatients.

ABOUT THE ROLE

We are currently recruiting for an experienced Social Worker at Holroyd Private Hospital, who can work casual with availability required on weekdays. The hospital provides Inpatient and outpatient Rehabilitation services to clients with a wide range of health issues in Orthopedics, Cardiac, Pulmonary, Neurological, and geriatric populations. The position is casual, with availability required on weekdays.

SKILLS AND QUALIFICATIONS:

· A bachelor’s or postgraduate degree in social work

· Excellent negotiation, decision-making, and advocacy skills including collaborative stakeholder consultation.

· Knowledge, skills, and clinical experience in Social Work clinical practices.

· The ability to work as a member of a multidisciplinary team.

· Good time management skills and ability to prioritize workload.

· Experience in Hospital and/or Aged Care SW preferred

RESPONSIBILITIES/DUTIES:

· Proactive Discharge planning including arranging services

· Liaising with community-based services

· Liaison with families in regard to discharge planning and assisting with aged care placements process.

· Working alongside senior SW and completing any delegated tasks assigned by them.
