About The Opportunity
An exciting opportunity exists for a Clinical Nurse Specialist Grade 2 – Patient Flow Access Nurse to join the team at Wagga Wagga Base Hospital.
In this senior clinical role, you will provide expert nursing leadership and coordination to support safe, timely and effective patient flow across the hospital. Working collaboratively with patients, families, nursing and medical teams and key hospital services, you will identify and address barriers to access, admission, transfer and discharge.
You will use your advanced clinical knowledge, communication and problem-solving skills to support the right patient receiving the right care, in the right place, at the right time.
In This Role, You Will
Provide advanced clinical nursing leadership to support safe, timely and effective patient flow across Wagga Wagga Base HospitalUndertake comprehensive clinical assessment and support appropriate patient placement, admission, transfer and dischargeIdentify and proactively address barriers to patient access, flow and timely dischargeCoordinate complex patient journeys in collaboration with medical, nursing, allied health and operational teamsProvide expert consultation, advice and support to clinical teams regarding patient flow, access and escalationAct as a clinical resource, mentor and support for nursing and multidisciplinary staffMonitor patient flow, identify emerging risks and implement strategies to optimise hospital capacity and patient outcomesPromote effective communication and collaboration across inpatient, emergency, community and support servicesLead and contribute to evidence-based practice, quality improvement and service development initiativesAdvocate for patients and families, ensuring care is coordinated, person-centred and delivered in the right setting at the right time

About You
Our ideal candidate will demonstrate:
Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered NurseDemonstrated clinical experience and advanced nursing knowledge relevant to patient flow, access and care coordination with relevant please write up at selection criteria questions post graduate qualifications Demonstrated ability to coordinate complex patient care and effectively manage competing priorities in a fast-paced environmentStrong clinical leadership, critical thinking, problem-solving and decision-making skillsExcellent communication, negotiation and collaboration skills, with the ability to work effectively across multidisciplinary teamsDemonstrated ability to identify barriers to patient flow and develop innovative solutions to improve access and patient outcomesCommitment to evidence-based practice, quality improvement, service development and ongoing professional developmentDemonstrated ability to provide clinical education, mentorship and support to nursing and multidisciplinary staff
