Aboriginal Health Practitioner
Full-Time | Geraldton, WA
Join the GRAMS team!
The Geraldton Regional Aboriginal Medical Service (GRAMS) is seeking a dedicated Aboriginal Health Practitioner to join our Geraldton team.
This rewarding role provides culturally secure healthcare for GRAMS clients while supporting the planning, coordination and delivery of quality primary health services. You'll work closely with GPs, nurses and other health professionals to support positive health outcomes for Aboriginal people.
Key Responsibilities
as an Aboriginal Health Practitioner, you will:
Provide culturally appropriate, holistic primary health care to GRAMS clients.
Triage patients and undertake opportunistic health screening prior to GP consultation.
Perform clinical duties including observations, wound care, blood collection, ECGs and immunisations under appropriate supervision
Assist with Adult Health Checks, 715 and other health assessments in collaboration with the Practice Nurse and GP.
Provide culturally appropriate health education and support chronic disease self-management.
Maintain accurate patient records and document clinical encounters in Communicare.
Work collaboratively as part of the GRAMS multidisciplinary clinical team.
What we're looking for
Current registration as an Aboriginal Health Practitioner with AHPRA.
Advanced Certificate in Aboriginal Health Work, associated Diploma in Aboriginal Health Work, or an accredited alternative qualification.
Knowledge of Aboriginal culture, health and related issues and ability to work effectively with Aboriginal people.
Strong communication and interpersonal skills.
Ability to work independently and within a multidisciplinary team.
Current C Class driver's licence.
National Police Clearance (or ability to obtain).
Working with Children Check (or ability to obtain).
