About the role
South West Aboriginal Medical Service (SWAMS) is seeking an experienced and dedicated Home Stretch Coach to join our team on a fixed-term 6-month contract.
The Transition Coach for Homestretch plays a vital role in supporting individuals in their journey towards independence and self-sufficiency. Based in Bunbury, Western Australia, this full-time contract position focuses on empowering clients through tailored coaching and guidance, ensuring they have the necessary skills and resources to thrive in their personal and professional lives. The role is essential in fostering a supportive environment that promotes positive change and growth within the community.
SWAMS is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. This position is designated under Section 50D of the Equal Opportunity Act 1984 (WA), and as such, only Aboriginal people are eligible to apply.
About the role:
This role provides support to Aboriginal young people by helping them access programs, services, and financial assistance, administering the Invest in Me Fund, and supporting successful transitions from Home Stretch WA. It maintains records, monitors and reports on outcomes, collaborates with key stakeholders, and participates in training and reflective practice activities. The role also upholds professional standards, complies with all SWAMS policies and legislative requirements, contributes to a safe workplace, and undertakes other duties as required.

Possession of or substantial progress toward a qualification in Social Work, Youth Work or other relevant qualification or substantial lived experience relevant to the position and willingness to complete further education.

All team members have a legal and moral duty to ensure a safe environment and are required to undergo a National Police Clearance. Roles involving direct contact with children also require a valid WA Working with Children Check as part of our pre-employment screening.
In addition to this, all applicants require satisfactory completion of a Drug and Alcohol test. Unsatisfactory pre-employment checks will result in the offer of employment being withdrawn.
