Applications are invited from suitably qualified medical practitioners for the above position in the Western Sydney Local Health District (WSLHD). This position will be located within Hospital in The Home (HiTH) as part of the Access to Geriatric Expert Services (AGES).
This is a Fixed Term appointment until 30 June 2028.
The Out of Hospital Care inTouch/HiTH AGES service operates across the Clinical Operations and Integration & Transformation Directorates within Integrated and Community Health and provides a district-wide service.
In addition to supporting clinical service delivery through Hospital in the Home (HiTH), inTouch and geriatric outreach teams, the AGES Geriatric Staff Specialist position(s) will work collaboratively with the Departments of Geriatric Medicine to develop a district-wide, integrated service model. This will include the development of a revised model of care, incorporating integrated referral pathways between existing Integrated and Community Health (ICH) services and facility-based geriatric and aged care outreach services.
Candidates for medical specialist positions in out-of-hospital care—such as home-based care, mobile clinics, or community-based specialist outreach—require a specific blend of advanced clinical skills, high adaptability, and strong patient-centred interpersonal abilities. They must be comfortable operating independently, often without immediate access to advanced diagnostic imaging or laboratory, while delivering high-quality, safe, and compassionate care in non-traditional settings.
In the context of healthcare system changes within Western Sydney local Health District — such as shifting from acute to out of hospital care models and increasing digital integration the ideal candidate will have a mix of traditional clinical expertise and advanced non-technical systems thinking—integrating knowledge, skills, and a holistic perspective—to improve care delivery, patient outcomes, and system efficiency. The successful candidate will need to act as change agent, demonstrating leadership, adaptability, and a commitment to patient-centred, collaborative care.
Essential Requirements for Appointment
1. Registered with the Medical Board of Australia and Fellowship of the Royal Australasian College of Physicians with appropriate experience to practice as a Senior Medical Practitioner in Geriatric Medicine “or equivalent as provided for in the Staff Specialists (State) Award/ Health Insurance Act 1997. Must have Australian Citizenship or Permanent Australian Residency.
2. Specialist Geriatrician with sound clinical knowledge and demonstrated recent experience in Geriatrics Medicine. Applicants with experience in community-based Geriatric Medicine, preferably as part of a multidisciplinary community-based team / as part of an outreach service, are highly regarded.
Conditions of appointment are in accordance with the Staff Specialists (State) Award.
Selection Criteria
1. 3. Proven ability to lead and adapt clinical practice in community environments.
4. Demonstrated ability to comply with the service objectives of public health organisations, including the provision of services to local community and effective utilisation of resources.
5. Demonstrated commitment to continuing self-education and development, including evidence of participation in continuing medical education/continuing professional development and service development and evidence of and commitment to quality improvement methodologies.
6. High level interpersonal and communication skills with leadership experience in managing change with a clinical service
7. Current NSW Driver’s license.
