Your Impact

At MMT Care, every action you take matters. As an experienced Support Worker, you'll play a vital role supporting individuals with complex care requirements, high physical support needs, and behavioural support needs, helping them live safe, fulfilling, and meaningful lives.

You will:

• Empower clients to achieve personal, social, and self-care goals, creating memories that last a lifetime.
• Support participation in community programs that bring joy, connection, and experiences worth remembering.
• Assist with daily living tasks including personal care, meal preparation, and nutrition support.
• Provide safe mobility assistance, including hoist transfers and manual handling.
• Support individuals with complex behaviours in a safe, respectful way.
• Deliver high-quality support to clients with complex care needs and high physical support requirements.
• Keep accurate records and progress notes, ensuring continuity of care and positive outcomes for clients.

About You

You're warm, approachable, and genuinely passionate about helping others create meaningful moments. You take pride in the quality of care you provide, are a strong communicator, and thrive both independently and as part of a team.

We're particularly interested in hearing from Support Workers who have experience supporting individuals with complex care needs and high physical support requirements.

What You'll Need

• Demonstrated experience supporting individuals with complex care needs and high physical support requirements (highly regarded)
• Current certifications: Level II First Aid and CPR, NDIS Worker Screening Check, and Working With Children Check/Blue Card (or willingness to obtain)
• Current Australian Driver's Licence
• Ability to support clients while creating memorable experiences and positive outcomes
