Personal Care Worker

About the Role

As the Personal Care Worker, you will develop non-clinical aspect of customer care, undertake case management for Community Care support programs and implement care services for customers and communities. You will be required to contribute to the development of policy initiatives and business strategies. Additionally, you will also maintain customer records, resolve customer issues and promote the culture of service delivery to improve lifestyle experiences of customers.

Responsibilities include:

• Develops non-clinical aspect of plans in relation to customer care, contributes to the development of policy initiatives, business strategies and planning processes.
• Ensures that the team members are following established safety protocols and emergency procedures. Drives adherence to confidentiality and privacy regulations regarding clients’ personal information and care.
• Undertakes case management for high level Community Care support programs, manages the quality management framework to ensure care standards are met and promotes a culture of service delivery that aims to improve the lifestyle experiences of customers.
• Contributes to the growth of the business by building business development opportunities, liaising with local networks, professionals, government departments and suppliers to promote the employer as well as remaining abreast of emerging issues.
• Ensures all care activities comply with organizational policies, safety standards, and regulatory requirements and participates in quality improvement initiatives and best practices for care delivery.
• Resolves complex customer and care issues which may cause blockages in meeting agreed targets/plans and customer outcomes.
• Ensures customer records, incidents and complaints are documented and maintained correctly and participates in any investigations as required.
• Conducts training for junior personal care workers and new joiners, shares knowledge and skills with colleagues and supports their development.

About You

To be successful in this role, our preferred candidate will have:

• Certificate IV in Individual Support (Ageing); or currently studying a Diploma of Nursing or Bachelor of Nursing with the completion of a successful clinical placement; or equivalent.
• Completion of Administer and Monitor Medications (HLTHPF007), and/or Provide Support to People Living with Dementia (CHCAGE005), and/or Deliver Care Services Using a Palliative Approach (CHCPAL003) is prescribed.
• Moderate experience (post Certification IV) in providing support to people living with Dementia.
• Proficient in Person Centred Practice, Medication Administration & Monitoring and Case Management skills.
• Excellent analytical thinking, communication, customer experience, emotional intelligence, planning and prioritisation and results orientation skills.
