We are seeking an experienced Registered Nurse to join our Warwick Aged Care Service as a Care Coordinator, playing a key role in coordinating high-quality, person-centred clinical care for our residents.

In this pivotal position, you will provide clinical leadership and oversight for residents with complex needs, ensuring care is safe, coordinated, and aligned with our Positive Wellbeing Model. You will work closely with the Service Manager and clinical team to support service quality, compliance, and continuous improvement.

About The Role

As Care Coordinator, you will:

Coordinate and oversee baseline and focused clinical assessmentsDevelop, review, and monitor individualised care plansEnsure accurate clinical documentation and AN-ACC data collectionSupport clinical programs, audits, and quality initiativesLiaise with external service providers to ensure seamless care deliveryProvide guidance and leadership to clinical staff, promoting a culture of safety and accountability

About You

You are a confident clinician ready to take the next step, with a strong focus on care quality, leadership, and resident wellbeing.

Essential

Registered Nurse (AHPRA registered)Minimum 3 years’ post-graduate experience (aged care or acute setting)Strong leadership, communication, and decision-making skillsCompetent with clinical systems and Microsoft OfficeCurrent CPR & First AidAbility to obtain required police and insolvency checksFull Australian working rights (as we are not offering sponsorship for this role)Postgraduate qualification in Gerontology, Dementia, or similarExperience using mobile clinical documentation systems
