Monash Health is seeking experienced Nurses and Allied Health Professionals to join our Aged Care Assessment Service (ACAS) Assessment, Triage and Reviews Team.
This is an exciting opportunity to contribute to the transformation of aged care assessment as Monash Health leads the delivery of the Vic 2 Region Single Assessment Service. You will help create a simpler, more accessible assessment experience for older people, connecting them with the care and support they need to remain independent and achieve what matters most to them.
About the Role
As an Aged Care Clinical Assessor, you will use your clinical expertise and sound professional judgement to undertake holistic assessments of older people, and younger people with age-related care needs.
Working across triage, comprehensive assessments, Home Support Assessments and support plan reviews, you will consider each person's physical, medical, functional, psychological and social needs. Importantly, you will also recognise their strengths, goals, preferences and potential for reablement.
In partnership with clients, carers and families, you will develop individualised support plans that promote independence and help people make informed decisions about remaining at home or transitioning to residential care.
Assessments may be undertaken in the community, hospital settings, by telephone or telehealth. The service operates Monday to Friday during business hours, with travel required across the Eastern and Southern Aged Care Planning Regions.
