About The Role

As a Support Worker, you will provide individualized, person-centred support to adults, young people, and children with disability and complex support needs.

You will work alongside participants, families, carers, and community networks to support individuals to achieve their goals while promoting dignity, independence, choice, and inclusion.

Key Responsibilities

Support participants with daily living activities and personal careAssist with household and domestic tasksEncourage independence and skill developmentSupport community participation and social inclusionImplement individual support plans and goalsFollow Positive Behaviour Support Plans (BSPs)Administer medication in accordance with policy and individual health plansAccurately complete progress notes and documentationBuild positive relationships with participants, families, carers, and stakeholdersEnsure participant safety, well-being, and safeguarding requirements are metPromote participant rights, dignity, and self-determination
