Street to Home provides support to people experiencing homelessness to obtain and maintain long-term accommodation while receiving holistic case management support.
As a Support Coordinator, you will work directly with individuals experiencing homelessness and complex life circumstances, supporting them to secure housing, improve wellbeing and build sustainable connections within their community. Working within Ruah's recovery-oriented Model of Care, you will deliver strengths-based case management, advocacy, service coordination and practical support to help clients achieve their goals and improve their quality of life.
You will proactively engage people who may be disconnected from support services, develop recovery plans, coordinate referrals, advocate for access to housing and community services, and work closely with Support and Peer Workers and external stakeholders to achieve positive outcomes.
This is a full-time max-term position until 30 June 2027.
