About us
Moreton ATSICHS operates a growing network of healthcare clinics for local Aboriginal and Torres Strait Islander people. Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. We bring together primary care, allied health, dental, mental health and wellbeing, maternity and early childhood, disability services, specialist services, and aged care as one team focused on clients' individual needs and goals.

About the role
As a Clinical Care Coordinator - MATSICHS Social Health, your role is to provide advanced clinical assessment, triage, and coordination of supports to Aboriginal and Torres Strait Islander clients experiencing complex social, emotional wellbeing and mental health challenges.
You will exercise high-level clinical judgement to assess risk, prioritise service responses, and deliver brief evidence-based interventions. The role works collaboratively with Social Health practitioners and provides clinical leadership in intake, triage, and care planning to ensure safe and effective client outcomes.
