Clinical Care Partner

This role does not carry line management responsibility; instead it operates as a trusted senior peer, the clinical reference point for the team when clients deteriorate or are complex and high risk to be supported at home. The Clinical Care Partner works closely with Care Partners providing timely advice and assessment and independently manages their own caseload of clients whose needs require ongoing clinical oversight and expertise.

What We're Looking For

• Current professional registration in one of: Registered Nurse (AHPRA); allied health professional (AHPRA, e.g. physiotherapy, occupational therapy); or social worker eligible for AASW membership via an AASW-accredited qualification
• Understanding of care planning and aged care funding programs
• Demonstrated skills in care coordination and communication
• Postgraduate study or demonstrated experience in aged, community or complex care; case management experience
• Knowledge of trauma informed and care practices and willingness to include Jewish care values into their everyday practice
