RSR Consulting Willow House Services is seeking an experienced and compassionate Community Worker to join its team in Wooloowin, Queensland. The successful applicant will work closely with individuals, families and community stakeholders to assess client needs, develop appropriate support strategies, coordinate access to community services and promote greater independence and community participation. This full-time role requires strong case management, communication and stakeholder engagement skills, together with the ability to support people experiencing a range of social, emotional, financial, housing, employment and other welfare-related challenges.

Task & responsibilities Interview clients and assess the nature and extent of their social, emotional, financial, housing, employment and other welfare-related needs and difficulties. Assess individual client needs and develop, implement and review individualised support strategies and community-based programs. Meet with clients to understand their circumstances, identify challenges and provide appropriate information, guidance and practical support.
Monitor and evaluate client progress and outcomes, maintaining accurate case notes, records and progress reports. Assist clients to identify and access appropriate health, welfare, housing, employment, education, training and other community services. Refer clients to government agencies, community organisations, specialist providers and other services where additional assistance is required.
Assess community needs and available resources relating to health, welfare, housing, employment, training and other support services. Liaise and maintain effective working relationships with community groups, welfare agencies, government bodies and other service providers to coordinate client support. Promote awareness of available community resources, programs and services and assist clients to access appropriate support.
Support families, young people and vulnerable community members to address social, emotional, financial and other personal challenges. Contribute to the planning, development and implementation of community programs that promote inclusion, independence, wellbeing and community participation. Collaborate with internal staff and external stakeholders to coordinate services and achieve positive outcomes for clients.
Prepare case documentation, progress reports, funding submissions and reports required by management, funding bodies and relevant government agencies. Qualification & experience Diploma of Community Services or an equivalent is highly desirable. Minimum one year of relevant experience in community services, social support, case management or a related field.
Demonstrated ability to assess client needs and develop appropriate support strategies. Experience providing information, guidance, practical assistance and referrals to individuals and families. Strong understanding of community services and available health, welfare, housing, employment and other support resources.
Strong communication, interpersonal and stakeholder relationship-building skills. Ability to work effectively with individuals and families from diverse backgrounds and circumstances. Strong organisational, case documentation, reporting and record-keeping skills.
Ability to work independently as well as collaboratively within a multidisciplinary team. Previous experience in community development, case management or community-based support will be highly regarded.
