About the role
As a Care Partner (Intake) for COHS, you will play a crucial role in supporting elderly participants in the South-west Sydney, NSW area. This full-time position involves assessing and planning clients' needs, coordinating care services, and ensuring they receive the support they require to live independently at home. You will be an integral part of the COHS team, dedicated to enhancing the quality of life for our community's most vulnerable members.
What you'll be doing
Conduct comprehensive participant intake assessments in collaboration with the participant to understand their unique needs, goals and preferences.
Develop, implement, monitor and review participant Aged Care Support at Home budget, schedule of supports and provide monthly statements.
Collaborate with participants, their families, and healthcare providers to develop person-centered care plans.
Coordinate and schedule a range of Aged Care (Support at Home) in-home support services, such as personal care, housekeeping, meal preparation and social activities.
Continuously monitor the participant's wellbeing and adjust care plans as needed.
Serve as the primary point of contact for participants and their families, providing compassionate support and guidance.
Maintain detailed client records and ensure compliance with all relevant regulations and policies.
Work closely with the wider COHS team to drive continuous improvement and enhance the quality of our services
What we're looking for
Care Manager (Arabic speaking preferred) who is experienced in Aged Care Support at Home (previously Aged Care Home Care Package program).
Demonstrated knowledge and understanding of the new Aged Care Act 2024, Aged Care Rules 2025 and Aged Care Strengthened Standards 2025.
Relevant experience in the aged care or disability support sector, with a demonstrated commitment to person-centered care.
Strong communication and interpersonal skills, with the ability to build trust and rapport with clients and their families.
Excellent organisational and time-management skills, with the ability to prioritise and multitask effectively.
Empathetic and compassionate nature, with a genuine desire to make a positive difference in people's lives.
Relevant qualifications, such as a Certificate III in Individual Support or equivalent.
A current driver's licence and access to a reliable vehicle.
