The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you a dynamic and compassionate Nurse leader looking to make a real difference in your community? Lemongrove Community Health Centre is seeking a highly motivated Clinical Nurse Unit Manager (NUM) to lead our dedicated team and drive the delivery of high-quality, patient-centred care.
As the Clinical NUM, you will play a pivotal role in the coordination of patient and client care, unit management, and clinical leadership to ensure the delivery of safe, effective and quality nursing services, while promoting the efficient use of resources. Leading the Community Health Care Nursing team, you will provide both clinical and operational oversight of community health services and support the delivery of integrated care that improves health outcomes for the local community.
A key focus of this role is facilitating client flow within the service, ensuring each client has a documented plan of care and that key milestones throughout their healthcare journey are achieved. You will manage clinical issues including workload, capacity, clinical concerns, and the escalation of matters as required. Key to your success will be demonstrated experience in leading and facilitating collaborative multidisciplinary care, ensuring coordinated care planning, streamlined service access, effective client flow, and successful referral and transition pathways between hospital, community and external service providers.
