The role focuses on promoting clients' independence, wellbeing, dignity and participation in their communities while supporting them to continue living safely and independently in their own homes.

Monitor and regularly review clients' progress and changing circumstances, evaluate the effectiveness of existing support arrangements, and recommend appropriate adjustments to individual care and support plans.
Coordinate the delivery of home and community-based support services in accordance with individual care plans and ensure services meet clients' assessed needs and agreed support goals.
Support clients to maintain social connections and participate in appropriate community activities, promoting independence, confidence and social inclusion.
Provide clients and their families or carers with information and guidance regarding available home-care services, support arrangements and service options.
Maintain accurate client records, assessments, case notes, care plans and progress reports and appropriately document changes in clients' circumstances and support requirements.
Identify concerns relating to clients' safety, wellbeing or ability to live independently at home and communicate relevant concerns to management and responsible members of the care team.
Contribute to the ongoing review and improvement of home-care programs and service delivery by identifying changing client needs, service gaps and opportunities to improve client outcomes.
About you
