About the role

As part of the Non-Family Based Care (NFBC) team you will coordinate care for children and young people under statutory orders placed in our 24/7 residential or Supported Independent Living Service (SILS). You will ensure clients have access to required supports, activities, development opportunities and living arrangements that are stable, safe, nurturing, trauma informed, affirming of culture, connected to community and which are age and stage appropriate.

Working closely with each child and young person (and others involved in their care, support and cultural identity) you will develop, implement and review Support Plans that are informed by their strengths, needs, preferences and aspirations. You will assist each child and young person to navigate their journey ‘into, through and out of’ placement ensuring their voice is central, that progress is made towards development milestones and or the transition to independent living and connection to ongoing supports.

Duties & Responsibilities

• Work collaboratively with the NFBC team to meet Child Safety related standards of care and licensing requirements, HSQF standards, Child Safe Organisation standards and the Universal Principle of cultural safety for children and young people in care.
• Establish rapport and engage positively with children and young people in care. Support their journey ‘into, through and out of’ NFBC through effective care coordination, stakeholder collaboration and integrated approaches.
• Coordinate care in ways that ensure children and young people are central to assessments and the development, implementation and regular review of Support Plans and that goals are informed by strengths, needs, perspectives and aspirations.
• Maintain good communications between clients, NFBC staff, Child Safety and stakeholders to ensure schedules, tasks, expectations and responsibilities are clear in relation to each child and young person’s care, support and development.
• Ensure each child and young person have access to family (parents, siblings, extended family members), friends, community supports, cultural connections and or specialist services as approved (and authorised) by Child Safety.
• Assist Residential Care Workers to apply positive behaviour strategies, culturally safe and trauma informed approaches and to embed household routines to sustain safe, respectful and inclusive home like living environments.
• As needed, provide practical assistance to enable each child and young person to achieve their Support Plan goals in line with the Child Safety Case Plan / Placement Agreement such as attending appointments and or school events etc.
• Provide professional supervision to Residential Care Workers in line with the Supervision Policy and Framework ensuring Supervision Agreements are signed, adhered to and that required records are maintained.
• Maintain good record keeping, data entry and report writing practices and ensure essential Child Safety related documents, case notes, client reports, support plans, risk assessments and incident reports are completed in a timely manner.
• Develop strong working relationships with Child Safety, Schools, First Nations entities, children and youth services sector and other specialist providers that can help meet the needs of children and young people in care.
• Facilitate care team, staff meetings and or stakeholder sessions as required and support NFBC management efforts to mitigate and manage risks, improve service design, embed contemporary practices, maintain quality and measure outcomes.
• Work cooperatively with NFBC management to ensure critical care tasks and operational needs are met.
• Assist management with client file reviews, service review and audits, support improvements in service design, implementation of innovations and efforts to maintain quality.

To be successful you will need

• Preference is for tertiary qualifications in Social Sciences. *Note a Certificate IV in Child, Youth & Family Intervention (or equivalent Child Safety approved course) combined with 5 years’ relevant experience may be considered.
• Strong record of working with children / young people from diverse cultural backgrounds in statutory care who have experienced trauma, serious adverse childhood experiences and or have complex care needs and an understanding of the child safety system.
• Ability to maintain professional boundaries and apply positive behaviour strategies, relational and trauma informed approaches combines with a commitment to ensuring children and young people are central to decision making and culturally safe.
• Prior experience in case management / casework / support coordination / needs assessment, the ability to navigate service systems and to develop, implement and review support plans and maintain stakeholder relationships.
• Demonstrated skills in risk assessment, responding effectively to crisis situations and incident reporting.
• Prior staff supervision and or line management experience.
• Demonstrated administrative, data entry, computer, report writing skills and the ability to learn new systems and data bases.
• Track record of strong Teamwork, collaborative practice and ability to guide the work of direct care staff.
• Commitment to diversity, inclusion, reconciliation and closing the gap objectives and experience working with First Nations people, organisations and communities.

Compliance

• Evidence of mandatory qualifications
• Satisfactory outcome of a current (less than 3 months old) National Criminal History Check
• Blue Card (QLD working with children card)
• LCS2 (child safety screening check)
• QLD drivers’ licence ‘Class C’ or interstate equivalent
• Ability (and willingness) to participate in a rostered (shared) 24/7 On Call system
• Required to transport clients and or to visit clients in their place of residence or within another out of office setting
• Out of hours work may be required from time to time
• Ability (and willingness) to undertake local area / regional travel
• First Aid certificate and CPR certificate
