Transition Registered Nurse-Identified

The Transition Registered Nurse (RN) in School Based Youth Health delivers safe compassionate clinical care, working collaboratively with the Healthcare team to achieve best practice client outcomes. The RN engages in reflective, analytical practice through effective assessment, planning, implementation and evaluation of direct person-centred care. The RN continually improves their skills and knowledge developing from graduate to highly skilled Registered Nurse.

About the Team

The School Based Youth Health Nurse Service aims to promote positive health outcomes for young people and their families through the provision of a primary health care service centred around the principles of early intervention, assessment and harm minimisation and prevention. The service provides, the opportunity for young people, their parents/ carers, and members of the school community to access a health professional in the school setting for matters relating to the health and well-being of young people

About the Role

In this role you will deliver quality outcomes for children, young people and families (Leader of Self) in accordance with the Children's Health Queensland Leadership Excellence Framework and the Queensland Public Service and the CHQ values.

You'll fulfil the responsibilities of this role in accordance with Schedule 2, Nurses and Midwives (Queensland Health) Award - State 2015 - Generic Level Statements (GLS) Nurse Grade 5 and the Queensland Health and Children's Health Queensland Hospital and Health Service (CHQHHS) commitments and values, in achievement of the organisational goals and Domains of Practice.
