Support Worker

Thrive West Pty Ltd is seeking a caring, reliable and capable Disability Support Worker to join our team in Merredin.

About the role

This is a casual position supporting people with disability to build independence, participate in their community and work towards their personal goals. Depending on the needs and preferences of each participant, your work may include supporting participants with daily living activities, providing assistance with community access, shopping and appointments, supporting participants to develop practical and social skills, transporting participants to activities and appointments, supporting participation in recreational and community activities, following individual support plans, risk assessments and behaviour support strategies, completing accurate case notes and other required documentation, and communicating professionally with participants, families and the Thrive West team. Shifts may be available during weekdays, evenings, weekends and public holidays.

Key responsibilities

• Supporting participants with daily living activities

• Providing assistance with community access, shopping and appointments

• Supporting participants to develop practical and social skills

• Transporting participants to activities and appointments

• Supporting participation in recreational and community activities

• Following individual support plans, risk assessments and behaviour support strategies

• Completing accurate case notes and other required documentation

• Communicating professionally with participants, families and the Thrive West team

About you

• Treats people with dignity, respect and patience

• Is reliable, punctual and able to work independently

• Has strong communication and interpersonal skills

• Understands professional boundaries and confidentiality

• Can remain calm and respond appropriately in challenging situations

• Is willing to follow participant support plans and workplace procedures

• Is comfortable using a mobile phone or computer to complete case notes

• Has a genuine interest in helping people achieve greater choice and independence

• Hold, or be willing and eligible to obtain: NDIS Worker Screening Check, National Police Clearance, Working with Children Check where required, Current Australian driver's licence, Completion of the NDIS Worker Orientation Module
