About the role
At The Health Lane, we connect with people because we understand and make everyone feel validated and safe. We support people of all ages with disability, embracing and fostering diversity, and we believe in social equity — assisting and empowering people to find their own path forward.
We're looking for an experienced and patient Support Worker to work one-on-one with a participant who has complex behaviours, helping them feel safe, understood, and supported to live their best life.
This isn't an entry-level role. We need someone who is calm under pressure, genuinely skilled in positive behaviour support, and committed to building a trusting, consistent relationship with the person they support.
About the Role
You'll provide skilled, individualised support to participants with complex behaviours of concern, working closely with each person's support network to deliver consistent, high-quality care in line with their Behaviour Support Plan.
Provide support tailored to each participant's individual support plan and Behaviour Support Plan
Recognise early warning signs and use proactive, de-escalation and positive behaviour support strategies
Support participants with daily living, community access, and personal goals
Build stable, trusting relationships through consistency and predictability
Work collaboratively with Behaviour Support Practitioners, families, and the broader care team
Maintain clear, accurate incident reports and shift documentation
Respond calmly and safely to challenging situations, following agreed strategies
Champion each participant's choice, dignity, and rights in every interaction
About You
Proven experience supporting people with complex behaviours or high-intensity needs (essential)
Certificate III or IV in Individual Support, Disability, or related field (or equivalent experience)
Training or genuine willingness to train in Positive Behaviour Support and restrictive practice awareness
Calm, resilient, and able to de-escalate difficult situations without taking things personally
Strong communication and documentation skills
Current NDIS Worker Screening Check (or willingness to obtain)
Current First Aid & CPR (or willingness to obtain)
Current Driver's Licence and reliable vehicle (mileage reimbursed)
