As a Supportive Care Case Manager, you'll support people and families impacted by blood cancer through compassionate, personalised care, via phone, video, face-to-face and digital channels.
It's a Monday to Friday role with flexible hours, no nights, weekends or on-call, and the freedom to plan your week across office-based work and hospital visits, backed by onsite training and ongoing CPD.
About the role
Reporting to the Clinical Services Manager, you'll manage a caseload of clients, patients, carers and their loved ones, delivering personalised, high-quality supportive care from pre-diagnosis through to survivorship and end of life. This includes comprehensive assessments, care planning, and regular outreach visits alongside phone, video and digital support.
You'll also build strong relationships with hospitals and health services, keeping the Leukaemia Foundation an accessible, visible presence in your region.
