About Us?

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

Operating within the Regional and Specialist Services, the purpose of the Staying Deadly Hub team is to provide comprehensive assessment treatment and assertive outreach support to clients presenting with moderate to severe mental health needs.  Working in a unique multidisciplinary care team as an extension of the IUIH comprehensive model of care, along with integration with the Staying Deadly Hub teams to provide appropriate and responsive care to clients would otherwise not access support or require impatient care.

About the Role?

As Social Health Care Coordinator your role is to provide coordination of supports to Aboriginal and Torres Strait Islander clients who experience complex social and mental health conditions and to work effectively with other social and health practitioners to identify and access appropriate services and supports to meet their needs.  This position is embedded in the IUIH Goodna clinic and works collegially with Counsellors, Psychologists and other Social Health team members along with the Family Wellbeing Service ensuring optimal client outcomes.

Your day-to-day duties will include:

• Process new referrals and provide Case Management via MMex
• Initial triage/assessment of referrals to aid allocation – via telephone or face-to-face as required
• Brief intervention provision
• Respond to crisis/acute presentations as available
• Maintain Social Health Register and manage Wait Lists for Social Health team
• Contribute to Weekly Intake Meeting and participate in client journey care reviews

About You?

• Current Criminal History Check
• Current C Class Drivers Licence (QLD)
• Current Working with Children’s Check (Qld Blue Card)
• Current CPR Certificate
• Minimum Diploma of Health Services / Social Service / Social Sciences or relevant qualification
• Minimum three years’ relevant experience
• Case management experience in a comparable role

Desirable, but not mandatory:

• Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
