About the Role

As a Support Worker, you will provide high-quality personal care and practical support to community-based customers in accordance with their individual care plans. Working collaboratively with a multidisciplinary team, you will help customers with daily living activities, monitor their wellbeing and create a safe, supportive environment that promotes dignity, choice and quality of life.

Key Responsibilities

You will bring a compassionate, patient approach and a genuine commitment to supporting positive outcomes for people with disability. Some of the key responsibilities expected are:

•  Supporting customers to attend appointments, shopping, social events and recreational activities.
• Encourage independence, confidence and community participation.
• Assist customers to build daily living and social skills in their personal home.
• Follow individual support plans, risk assessments and workplace procedures.
• Complete clear and accurate case notes and incident reports.
• Maintain professional boundaries and respect customer privacy, dignity and choice.

Essential Requirements

• A genuine interest in supporting people with disability in the community.
• A respectful, patient and person-centred approach.
• Good communication and time-management skills.
• Ability to work independently and as part of a team.
• Understanding of working with digital apps and online systems.
• NDIS Worker Screening Check.
• Working with Children Check.
• Current First Aid and CPR certificates.
• Current driver’s licence and access to a reliable, insured vehicle.
• Australian working rights.
• Experience with hoists, seizure management and dysphagia preferred

Previous disability support experience or a relevant qualification, such as a Certificate III in Individual Support, is desirable but not always essential.
