Case Worker – Early Intervention and Family Support (50D)

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper Wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

About the role

The Case Worker will support the delivery of the Regional Support Hub Bindjareb program by working directly with Aboriginal children, young people, and families requiring assistance. As a key point of contact within the community, the role will provide early intervention, advocacy, referral, and support services to help families access the resources they need.

The Case Worker will build and maintain strong partnerships with local service providers, develop and strengthen support and interagency networks, and facilitate referrals to ensure clients can effectively navigate available services. The role will support culturally appropriate service delivery, maintain accurate client records, and contribute to positive outcomes for children and families. This role also travels to outreach sites.

Additionally, the Case Worker will assist in managing the Service Network phone service, connecting clients with appropriate supports and services in a timely and effective manner.

Essential Criteria:

• Aboriginal or Torres Strait Islander descent in accordance with Section 50(d) of the Equal Opportunity Act.
• An understanding of the Children and Community Services Act 2004 and its application when working with children and families.
• Knowledge of the developmental needs of children and young people, with a commitment to supporting their wellbeing and safety.
• A strong understanding of Aboriginal culture, values, and the contemporary issues impacting Aboriginal people, with the ability to build respectful and trusting relationships with children, families, communities, and stakeholders.
• Excellent written communication skills, including the ability to prepare clear, accurate, and meaningful reports.
• Strong organisational and time management skills, with the ability to manage competing priorities effectively.
• A collaborative approach and the ability to contribute positively as part of a supportive team.
• The ability to build positive partnerships and work confidently with government and non-government agencies, community groups, and service providers to achieve the best outcomes for families.
• Well-developed problem-solving, analytical, and case management skills, with a strengths-based approach to supporting children and families.

Desirable Criteria:

Possession of or substantial progress toward a qualification in Social Work, Youth Work or other relevant qualification or substantial lived experience relevant to the position and willingness to complete further education.
