Use Your Expertise to Strengthen Mental Health Care Pathways

Permanent full-time opportunity for an experienced Senior Allied Health professional to provide specialist clinical care, leadership and care coordination across Metro South Health and The Way Back Support Service.

As the Clinical Care Coordinator, you will provide high-level discipline-specific clinical services and coordinate care pathways for people experiencing suicidal crisis or who have attempted suicide. Working collaboratively with multidisciplinary teams and The Way Back Support Service, you will help ensure consumers receive coordinated, appropriate and recovery-focused support across clinical and non-clinical settings.

In this role, you will:

• Deliver evidence-based clinical services in social work, occupational therapy or psychology, including assessment, intervention and evaluation.
• Coordinate referrals, clinical care and communication between Metro South Health and The Way Back Support Service.
• Provide clinical leadership, education, supervision and support to less experienced clinicians, students and support staff.
• Build strong partnerships with internal teams and external service providers to support seamless care pathways and positive consumer outcomes.

We are looking for an experienced Senior Allied Health professional with strong clinical expertise and a passion for improving outcomes for people experiencing complex psychosocial needs. You will bring sound clinical judgement, excellent communication skills and the ability to work effectively across multidisciplinary and community-based services.

You will bring:

• High-level clinical knowledge and experience in psychology, social work or occupational therapy, preferably within mental health services.
• Demonstrated ability to manage complex clinical presentations, prioritise workloads and coordinate effective care and discharge planning.
• Strong communication, negotiation and relationship-building skills, with the ability to work collaboratively with multidisciplinary teams and external stakeholders.
• Demonstrated leadership skills, including providing clinical advice, supervision, education and contributing to service improvement initiatives.
