Disability/Aged Care Support Worker

About the role

As a Support Worker at Eminence Care, you will provide one-on-one in-home support to clients with daily living activities and community participation. You will work across Melbourne supporting individuals to maintain independence and enhance their quality of life.

Key responsibilities

• Providing one-on-one in-home support to clients with daily living activities such as washing, dressing, grooming, meal preparation, and transport assistance
• Supporting community participation, social activities, and outings
• Assisting with domestic tasks, including cooking, cleaning, and shopping
• Encouraging independence and self-confidence through tailored support programmes
• Reporting and recording client information in line with NDIS requirements
• Supporting clients with hoist transfers and manual handling as required

About you

• Certificate III in Individual Support qualification is mandatory for this position
• Minimum of 6 months experience in disability or aged care services
• Knowledge and awareness of the National Disability Insurance Scheme (NDIS)
• Experience with behaviour management, hygiene care, peg feeding, hoist/manual handling, and community engagement
• Strong communication, time management, and organisational skills
• The ability to work independently and as part of a team
• A client-focused, safety-first approach with attention to confidentiality and professionalism
• Valid work visa or Australian work rights are required
