We are looking for an experienced Care Manager to join our leadership team at well established home Pembroke Lodge in Minto.

SKILLS AND EXPERIENCE

Current registration with AHPRA, Division 1

Five (5) years post general nursing experience

Five (5) years experience in residential aged care

Residential aged care management experience as CM or CCC

Excellent communication skills

Demonstrated Leadership

Great work ethics

Certificate/ Diploma in Gerontology/ Aged Care/ Management Preferred

MAIN RESPONSIBILITIES

 To assist the Facility Manager in achieving company goals & objectives

 To assist the Facility Manager in the provision of management services and operational direction for all employees.

 To be responsible for ensuring the smooth and efficient operation and provision of care services.

 To act as clinical consultant for personal and health/ clinical care matters.

 To monitor Health and Personal Care Standards are maintained for residents.

 To assist the Facility Manager to ensure employees are adhering to company policies and procedures in all areas of service delivery.

 Good understanding of AN-ACC funding instrument

 IPC Lead preferred

SALARY
