As an experienced Support at Home Care Partner, you will work collaboratively with the DPV Health service teams. Your guidance and direction will assist our participants, their carers and representatives with the development and implementation of care plans that are flexible, personalised, and culturally appropriate. In this team, you;
Review intake information, engage new participants, establish Support at Home agreements, care plans, budgets and connect our participants with local services and community resourcesConduct participant visits practicing person centred care planning, provide independent advice to participants and their families - enabling personal preferences and independence at homeInteract with DPV Health team members, stakeholders, partnered service providers representing the best interests of participants.Conduct annual reviews for all participants - revise care plans, goals, service provisions to quality standards and budget expenditure and maximisationEnsure service provisions are conducted ethically, fairly and within the statutory legal and contractual requirements at all timesWork from various sites
To be considered for this rewarding role you will have:Diploma level or above qualification in community, aged, health or another discipline desirableExtensive case management experience (1 year minimum)Knowledge of & enthusiasm for Community Aged Care Services & Consumer Directed care environmentDemonstrated experience in working with ACAT/ RAS (SAS – Single Assessment Team), Care Finders and other referrersStrong experience in managing participant budgetsHigh level problem solving, exceptional customer service skills and analytical skillsExcellent IT skills, including Microsoft Office suite
The fine printSuccessful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.Full driver licenseLevel 2 First Aid Certificate
