About the role
Deliver vital nursing care, clinical assessment, and education to patients across the hospital's Diabetes Centre, Coordinated Care Clinics, High Risk Foot Clinics, and inpatient wards.

• Provide clinical nursing care to diverse patients with type 1, type 2, and complex diabetes conditions
• Partner closely with endocrinologists, dietitians, and diabetes educators in a high-volume setting
• Conduct clinical assessments and assist with complex insulin management plans
• Help patients prevent long-term complications and manage their health effectively
• Coordinate care pathways, acute triage, & community referrals for a culturally & socially varied patient population

About you

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
• Demonstrate a minimum of 2 years post graduate experience and demonstrated clinical experience in acute diabetes care and chronic disease management
• Experienced in working within a multidisciplinary healthcare team in high-volume clinical setting
• Working knowledge of continuous glucose monitors (CGM) and insulin delivery systems
• Strong interpersonal skills to support patients from diverse socio-economic backgrounds
