Speech Pathologist, Transition Care Program

Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.

• Permanent Part Time - 48 hours per fortnight

• Classification: Speech Pathologist Grade 2 Year 1 - Year 4 (VW1 - VW4)* plus super

The Team & Role

Join Bayside Health's Transition Care Program (TCP), a multidisciplinary service supporting older adults to transition safely from hospital to home across the Mornington Peninsula. As the Grade 2 Speech Pathologist, you will manage a diverse community-based caseload, working closely with Nursing, Physiotherapy, Occupational Therapy, Dietetics and Social Work to deliver evidence-based, person-centred care. While this is a sole clinician role within TCP, you will be professionally supported by a broader Speech Pathology team committed to clinical excellence, continuous improvement and student education.

Skills & Experience Required

• Eligible for practising membership of Speech Pathology Australia.
• Sound experience in the provision of best/evidence based adult related speech pathology.
• Strong communication and partnership skills with clients, families/carers and health professionals.
• Well-developed communication and interpersonal skills
• Knowledge of / experience in a community settings.
