Nurse - Care Coordinator

KinBridge Care is a young, nurse-led home care organisation on a mission to help people stay safe, independent and connected in their own homes. We're growing fast across Aged Care, DVA and NDIS services in South-East Queensland — and we're just getting started (Queensland-wide and beyond is the plan!).

We're looking for an Enrolled or Registered Nurse – Care Coordinator who wants more than "just a job." We want someone who's genuinely excited to help build something from the ground up, bring fresh ideas to the table, and grow with us as we expand. If you like variety, autonomy, and being part of a small team where your voice actually matters, this is for you.

The Role

You'll coordinate care from the ground up — client intake, rostering, nursing assessments, care planning and clinical support — reporting to our Clinical Services Manager. You'll work within your scope of practice, build great relationships with clients, families and staff, and help us keep raising the bar on quality care.

You'll be across things like:

• Client intake, assessments and person-centred care planning

• Nursing care, clinical coordination and escalation of clinical risks

• Rostering and scheduling via CareLineLive

• Liaising with GPs, hospitals, allied health and other providers

• Supporting and coaching frontline care staff

• Helping us stay compliant and continuously improve as we scale

What You Bring

• Enrolled or Registered Nurse with current, unrestricted AHPRA registration

• 3+ years' nursing experience (aged care, community, disability or primary care highly regarded)

• Solid understanding of person-centred care, clinical risk and documentation

• Comfortable with digital systems (CareLineLive or similar a bonus)

• National Police Check, NDIS Worker Screening Check, current First Aid/CPR

• Driver's licence and reliable vehicle

• A collaborative, can-do attitude and genuine enthusiasm for helping a growing organisation create real value for the community.
