As a Clinical Care Coordinator, you will be the clinical heart of our expanding service, ensuring high-quality, compliant care across our growing community.

• Lead Care Planning: Conduct home assessments and design individualised care plans that empower independence.

• Mentor & Supervise: Provide specialist nursing advice and leadership to our dedicated, growing team of care staff.

• Hands-On Clinical Care: Manage wound care, medication administration, and specialised treatments.

• Coordinate Continuity: Act as the vital link between clients, families, hospitals, and allied health professionals.

• Advocate: Support NDIS and My Aged Care compliance to ensure every client hits their personal goals.

• Credentials: Current AHPRA Registered/Enrolled Nurse Registration is essential.

• Clinical Savvy: Strong foundational nursing skills with a "quality-first" mindset.

• Communication: Exceptional verbal and written skills—you’ll be building relationships every day.

• Tech Literacy: Proficient in Microsoft Office and Excel.

• Mobility: A valid Australian Driver’s License and access to your own reliable vehicle.
