We are seeking an experienced and passionate Homeless Outreach Workers to join our Housing and Homelessness team, delivering the Homeless Outreach Access Program (HOAP) and Immediate Housing Response (IHR) initiatives.
This rewarding role provides an opportunity to make a genuine difference in the lives of people experiencing homelessness, rough sleeping, housing insecurity and other complex challenges.
About the RoleThe Homeless Outreach Worker provides assertive outreach, assessment, brief intervention, advocacy, service navigation and coordinated support to individuals and families experiencing homelessness or housing instability throughout the Moreton Bay region.
Working across our sites at Lawnton, Redcliffe and Kallangur on a morning and afternoon weekly shift rotation - 8AM-4PM & 12PM-8PM within a strengths-based, trauma-informed, culturally safe and person-centred framework, you will:
Undertake intake, assessment and brief intervention activities with individuals and families experiencing homelessness or housing instability.
Develop practical support plans that address immediate needs and longer-term housing goals.
Provide advocacy, information, referrals and practical assistance to improve access to housing, health, legal, financial and community services.
Conduct assertive outreach to engage people experiencing homelessness, rough sleeping or housing instability.
Support clients to navigate housing systems and community support networks.
Coordinate warm referrals and active handovers to specialist and longer-term services.
Work collaboratively with government agencies, community organisations and specialist service providers.
Maintain accurate case notes, client records and reporting requirements.
Participate in outreach rosters, including afternoon shifts where required.
Qualification in Social Work, Human Services, Community Services or a related discipline, or an equivalent combination of qualifications and relevant experience.
Experience supporting individuals and families experiencing homelessness, housing instability or complex social needs.
Experience delivering outreach, crisis intervention, case management or brief intervention services.
Strong assessment, engagement and service navigation skills.
Knowledge of homelessness services, housing pathways and community support systems.
Exceptional communication, advocacy and stakeholder engagement skills.
Ability to work autonomously while contributing positively to a collaborative team environment.
Competency using client management systems and Microsoft Office applications.
Commitment to strengths-based, trauma-informed and culturally safe practice.
Mandatory Requirements
Current Queensland Driver Licence.
Current Working with Children Check (Blue Card) or ability to obtain.
National Criminal History Check.
Right to work in Australia.

1. Homelessness and Housing Support Practice Describe your experience supporting individuals and families experiencing homelessness, housing instability or complex social needs.
2. Assessment, Brief Intervention and Service Navigation Provide examples of your experience undertaking assessments, developing support plans, delivering brief interventions and coordinating referrals to appropriate services.
3. Outreach and Client Engagement Demonstrate your ability to engage vulnerable individuals and families using strengths-based, trauma-informed and person-centred approaches.
4. Housing Systems and Community Services Knowledge Outline your knowledge of housing pathways, homelessness services and community support networks, and how you have successfully navigated these systems for clients.
5. Stakeholder Engagement and Collaboration Provide examples of how you have built and maintained effective relationships with government agencies, community organisations and specialist service providers.
6. Administration, Reporting and Digital Systems Describe your experience maintaining accurate case records, meeting reporting requirements and using case management systems or databases.
7. Values-Based Practice Demonstrate your commitment to trauma-informed, culturally safe, strengths-based and inclusive practice when working with diverse and vulnerable communities.
