Client Liaison

We are currently seeking a compassionate and customer-focused Client Liaison Officer to join our Hervey Bay Aged Care at Home team in a permanent part-time capacity.   As the primary point of contact for many of our clients and their families, you will play a vital role in ensuring older Australians receive the services, support and care they need to continue living well at home.   About the Role Reporting to the Care Partnering Lead, you will be responsible for coordinating client care and providing exceptional service throughout the client journey. You will support new and existing clients through intake, assessment, care planning, service coordination and ongoing reviews, helping to deliver a seamless and positive care experience.   Key Responsibilities Conduct client intake conversations, telephone assessments and service enquiries Assist with care planning, reviews and ongoing evaluation of client needs Coordinate and schedule services to support continuity of care Act as the primary contact for client, family and representative enquiries Build and maintain strong relationships with clients, families and internal stakeholders Monitor service delivery and identify changing client needs or risks Maintain accurate records, documentation and system updates Support onboarding processes for new clients Contribute to quality improvement initiatives and service excellence Ensure compliance with organisational policies, aged care standards and program requirements  About You You are a caring and organised professional who enjoys building relationships and making a positive difference in people's lives.
