healthAbility, as the name suggests, plays a unique and valuable role in the health system. We work to address the root causes of vulnerability, prevent disease and promote lifelong health outcomes, empowering people in our community to live their best life.
Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The Opportunity
We are looking for a Partnerships and Growth Lead to join our supportive and professional multidisciplinary teams in a full time, 6-month project role.
Responsibilities include but are not limited to;
• Build and grow strategic referral partnerships across aged care, community health and allied health sectors.
• Identify and convert new business development opportunities that support organisational growth.
• Strengthen and grow healthAbility's provider network by analysing and evaluating performance and key metrics, identifying partnership opportunities, and building effective relationships that support high-quality service delivery and future growth.
• Design and embed referral management systems, tools and partnership frameworks that strengthen client pathways, support frontline teams and deliver sustainable growth outcomes
• Develop and manage a pipeline of partnership opportunities and referral sources.
• Implement effective CRM, referral management and reporting frameworks.
• Establish and expand CHSP and Support at Home partnership arrangements.
• Create business development collateral and stakeholder engagement resources.
• Strengthen provider networks to enhance service delivery and growth outcomes.
Skills and Experience
• Proven experience in business development, partnership management, stakeholder engagement and driving growth outcomes.
• Demonstrated success building referral pathways, strategic partnerships, and strong stakeholder relationships.
• Tertiary qualification in Business, Marketing, Health, Community Services or a related discipline, or an equivalent combination of qualifications and significant experience in business development, partnership management, stakeholder engagement, or growth-focused roles.
• Strong commercial acumen with the ability to identify opportunities, analyse market trends, and translate insights into action.
• Experience using CRM systems (such as HubSpot, Salesforce or similar) to manage partnerships, referrals, customer relationships and pipeline activity.
• Excellent communication, influencing, project management and organisational skills, with the ability to work autonomously and manage competing priorities.
• Experience within aged care, community health, disability, health or human services sectors, with knowledge of Support at Home, CHSP or similar government-funded programs highly regarded.
• Established industry networks and experience developing partnership materials, stakeholder communications, or business development collateral will be highly regarded.
• Current Victorian Driver's Licence.
