Kyabram District Health Service (KDHS) is a medium-sized rural health service that delivers a range of programs and services including acute, subacute, residential aged care and allied and primary health care services. KDHS has three campuses located in the townships of Kyabram, Stanhope and Tongala.

About the Role
A great opportunity for a Registered Nurse to work as a District Nurse to work independently within the community with the support of a dedicated and multi-disciplinary team.
The District Nursing Service, including Hospital in the Home provides holistic care to people in their homes and clinic settings aimed at providing person centred care, ensuring patients receive treatment at home that keeps hospital admissions and readmissions to a minimum. The District Nurse empowers patients and their carers to develop goals and be actively involved in their own care.
About You
To be successful in this role you will need to be a registered nurse with experience nursing in a clinical environment and/or a community setting, particularly relating to wound care, palliative care, gerontology, continence, community services and an understanding of aged care issues. You will need to be self-directed with good time management, communication and interpersonal skills.
You will also need to demonstrate that you can provide high quality care in a complex and changing environment as well as experience and confidence using information technology, including mobile devices.
