About the role

Based in Adelaide CDB, we are looking for a full time Discharge and Transition Clinical Care Coordinator who will be responsible for overseeing and monitoring clinical services to ensure ongoing care needs and meet KPIs.

You will be the primary point of contact in supporting clients and staff to ensure services are appropriately planned and transitioned and that completed care is reported and summarised back to referrers.

Your main duties will include:

• Identify clients in the clinical programs that are eligible for other Government funding streams and liaison with the referrer, MRU or healthcare professional for transfer of care.
• Monitoring long-term clients and care transition for new and existing clients of the SA Health Community Care Program.
• Completion of discharge summaries in line with contractual and clinical KPIs

About you

• Diploma of Nursing 
• APRHA Registration
• Experience in Sub-Acute/Acute Nursing
• Community nursing experience
• Full Drivers Licence
• Previous experience managing service contracts.
• Previous experience managing staff.
• Strong problem solving and time management skills.
• Effective communication skills
• Computer skills
• Ability to work over 7 day roster
• Logistical experience (desirable)
