Community Carers NSW is growing, and we’re looking for an experienced Support Coordinator to join our friendly team in Caringbah.

The Role

📍 Caringbah, NSW
⏰ Casual with an opportunity to transition to permanent employment for the right candidate.
💰 Hourly rate of $50–$70 (depending on experience)

What You’ll Do

• Coordinate NDIS supports and services

• Build strong relationships with participants and families

• Liaise with support workers and allied health professionals

• Monitor participant goals and service outcomes

• Maintain accurate records and case notes

• Support new participant onboarding

• Help participants access appropriate supports
  

About You

We’re looking for someone who has:

• 2+ years’ NDIS Support Coordination or Case Management experience

• Strong understanding of the NDIS

• Excellent communication and organisational skills

• Strong relationship-building and problem-solving skills

• Ability to work independently and as part of a team

• Level 1 or Level 2 Support Coordination experience is highly regarded
  

Essential

• NDIS Worker Screening Check

• Working With Children Check

• First Aid & CPR

• Driver Licence & reliable vehicle

• National Police Check or willingness to obtain

• Full Australian working rights
