About The Role
Due to continued growth, we are seeking a motivated Support Coordinator to join our team.
As a Support Coordinator with Propel, you will work closely with participants and their families to implement NDIS plans, build capacity, and coordinate effective supports to help them access services through the NDIS.
We are committed to developing our team to deliver high-quality outcomes for participants from diverse backgrounds, and we welcome applications from experienced Support Coordinators or those actively pursuing professional development in Support Coordination who are eager to grow with our organisation.
You will use a person-centred approach to:
Develop and strengthen participant support networks aligned to short-term and long-term goals
Build participant capacity to understand and self-direct their NDIS plans
Facilitate communication between participants and service providers
Assist with plan reviews, variations and service adjustments as required
Monitor budgets and ensure effective utilisation of participant funding
Ensure timely and accurate completion of all documentation, reporting, billing and compliance requirements in line with organisational and NDIS standards
We are currently seeking casual employees and offer flexible employment options. While these positions are currently casual, part-time or full-time opportunities may become available in the future. We are committed to internal progression and regularly provide opportunities for casual staff to transition into permanent roles as they grow with Propel.
Requirements
Minimum 2 years experience in Support Coordination or a similar role with transferable skills
Experience supporting people with disability and liaising with service providers
Strong understanding of the NDIS framework and plan implementation processes
Strong written and computer skills for the creation of reports and other documentation
Strong time management, prioritisation and problem-solving skills
Ability to work independently and collaboratively within a team environment
Current National Police Check
Working with Children Check
NDIS Worker Screening Check
Completion of the NDIS Worker Orientation Module
Current Australian driver’s licence
Desirable
Demonstrated experience working as a Specialist Support Coordinator managing complex situations including housing, behavioural concerns, mental health, forensic involvement, Child Protection and Family Services or other significant matters
Relevant tertiary qualification in Social Work, Disability, Individual Support or Community Services
Current First Aid and CPR Certificate
