About the role
Sunrise2Sunrise is seeking an experienced and motivated NDIS Support Coordinator to join our Support Coordination team on a casual basis.
We are looking for someone who can commit to a minimum of three days per week and is interested in developing their caseload, with the opportunity for the position to grow into permanent part-time employment as caseload and service requirements develop.
This is an excellent opportunity for an experienced Support Coordinator who enjoys working independently while being part of a collaborative team and wants the flexibility of casual employment with the potential for longer-term growth.
As a Support Coordinator, you will work alongside participants to help them understand and implement their NDIS plans, build capacity and connect with the services and supports they need to achieve their goals.
You will:
Manage an allocated participant caseload and provide person-centred, responsive Support Coordination.
Support participants to understand and implement their NDIS plans, funding and available supports.
Monitor plan utilisation, participant outcomes and funding to identify service gaps, under-utilisation, funding pressures or changing support needs.
Coordinate NDIS-funded, mainstream, community and informal supports and develop effective relationships with participants, families, providers and other stakeholders.
Prepare high-quality implementation, progress, reassessment and change in circumstances reports and coordinate supporting evidence where required.
Monitor upcoming plan reassessments and funding requirements and ensure key participant actions and reporting deadlines are proactively managed.
Work collaboratively with allied health professionals, Behaviour Support Practitioners and other providers to support participants with complex or changing needs.
Maintain accurate, timely and professional progress notes, participant records and claimable activity.
Identify and appropriately escalate safeguarding concerns, service delivery risks and significant changes in participant circumstances.
You will be an organised, participant-focused Support Coordinator who is confident managing your own caseload and understands the importance of proactive plan and funding management.
You will ideally bring:
Previous experience in NDIS Support Coordination, case management or a comparable coordination role.
Strong working knowledge of the NDIS, plan implementation, funding and support pathways.
Experience supporting participants with complex or changing support needs.
An understanding of Behaviour Support Plans, positive behaviour support and regulated restrictive practices, and experience collaborating with Behaviour Support Practitioners.
Strong report-writing, documentation and case management skills.
The ability to effectively manage competing priorities, participant appointments, reporting deadlines and claimable activity.
Strong professional communication and stakeholder engagement skills.
A commitment to participant rights, choice, control, supported decision-making and capacity building.
Relevant qualifications in Community Services, Disability, Mental Health, Social Work, Allied Health, Human Services or a related discipline are highly regarded.
Applicants must be available to work a minimum of three days per week.
We are looking for someone who wants to establish themselves within the team and build a sustainable participant caseload. Subject to caseload growth, performance and operational requirements, there is an opportunity for the position to progress from casual employment to a permanent part-time arrangement.
Some travel will be required for participant home visits, community appointments, stakeholder meetings and other participant-related activities.
The successful candidate must hold and maintain:
Current NDIS Worker Screening Clearance.
Current Working with Children Check – Employee.
Satisfactory National Police Check and International Police Check where applicable.
Current legal entitlement to work in Australia.
Current Australian Driver Licence and access to a reliable vehicle.
NDIS Quality and Safeguards Commission Worker Orientation Module – Quality, Safety and You.
All mandatory Sunrise2Sunrise screening, induction, training and compliance requirements applicable to the position.
You will join an established Support Coordination team with access to leadership, practice guidance and opportunities for professional development.
