About the Role
Hospital Pharmacy offers a dynamic, fast-paced environment where precision and collaboration are key, and your decisions will have a direct impact on patient outcomes
  
Hollywood Private Hospital is Western Australia's largest private hospital, offering pharmacists exposure to a wide range of specialties, complex clinical presentations and multidisciplinary healthcare teams. You will be based onsite, with paid parking available.
  
Responsibilities:

• Conduct medication chart reviews and medication reconciliation.
• Coordinate patient medication requirements for inpatient stay, day infusions and discharge.
• Educate and advise staff and patients on the safe and effective use of medicines.
• Develop and maintain professional relationships with medical, nursing and allied health teams.
• Dispense and supply medications and pharmacy products.
• Support patients, carers and hospital staff.

About You
You are a Pharmacist who thrives in a fast-paced, collaborative clinical environment and brings a high level of professionalism, attention to detail, and a patient-centred mindset. In this specialised setting, high-pressure situations are not uncommon, so strong communication skills and the ability to prioritise effectively are essential.
  
Essential requirements:

• General pharmacist registration with AHPRA, with no restrictions on practice.
• Minimum 1 year of post-registration pharmacist experience in Australia.
• Australian working rights, valid for at least 12 months.
• Ability to commit to the roster requirements of the position.
