What you will be doing:As a Support Coordinator, you will work alongside participants, families, carers and key stakeholders to:Help participants understand, implement and make the most of their NDIS plansCoordinate and connect participants with the right NDIS, mainstream and community supportsBuild strong provider relationships and ensure supports are working together effectivelySupport participants to build capacity, independence, social and community participationMonitor plan utilisation, funding and participant outcomesIdentify challenges and work proactively towards practical solutionsMaintain accurate documentation and manage caseloads and billable coordination hours effectivelyAs a Psychosocial Recovery Coach, you will additionally:Build recovery-oriented relationships grounded in hope, trust and respectWork collaboratively with participants to develop and review recovery goals Coach participants to build resilience, motivation, decision-making and practical recovery skillsSupport participants to navigate the NDIS and connect with mental health and community services Strengthen confidence, self-advocacy, social connection and independence
What we are looking for:We are interested in the person behind the résumé as much as your qualifications.You will ideally bring:Experience in NDIS Support Coordination, Psychosocial Recovery Coaching, Case Management, Mental Health or a related roleStrong working knowledge of the NDIS, NDIA processes, plans, funding categories and service systemsA sound understanding of psychosocial disability, recovery-oriented practice and person-centered supportStrong communication and relationship-building skills, with the ability to work confidently with participants, families and stakeholdersStrong case management, documentation, administration and computer skills, including Microsoft Office and client management systemsA solution-focused, strengths-based approach and a genuine commitment to participant choice and outcomesRelevant tertiary qualifications in Social Work, Psychology, Mental Health, Allied Health, Community Services, Case Management or a related discipline are desirableA Cert IV in Mental Health with substantial relevant experience, or equivalent skills and experience, will also be consideredCurrent Australian Driver Licence and access to a reliable vehicle

Mandatory screening: Successful candidates will be required to hold or obtain a NDIS Worker Screening Check and Working with Children Check.
