About the role
We are seeking a Day Program Team Leader to join Heart and Home Care in Bella Vista, NSW. This is a casual position that offers flexibility whilst making a meaningful impact on the lives of individuals we support. As Day Program Team Leader, you will play a crucial role in overseeing daily program activities, ensuring participants receive engaging, person-centred support, and leading a team to deliver quality outcomes. This role is integral to our aged and disability support services, helping us maintain our commitment to empowering individuals and enhancing their wellbeing through structured, purposeful day programs.
Key responsibilities
Lead and supervise day program activities, ensuring they are engaging, inclusive, and tailored to participant needs and preferences
Coordinate with team members to deliver high-quality support and care throughout the program day
Assess participant wellbeing and monitor engagement, reporting on outcomes and any concerns
Implement individual care plans and support strategies in accordance with participant goals and organisational policies
Create and maintain a safe, welcoming and stimulating environment for all participants
Manage program schedules, resources, and equipment effectively
Communicate with families, carers, and relevant stakeholders regarding participant progress and program activities
Maintain comprehensive records and documentation as required by legislation and organisational standards
Support the professional development and wellbeing of team members
Ensure compliance with workplace health and safety requirements and organisational procedures
What we're looking for
Experience in community services, aged care, or disability support settings with demonstrated leadership or supervisory experience
Strong understanding of person-centred care principles and commitment to delivering individualised support
Excellent communication and interpersonal skills with the ability to work effectively with participants, families, team members, and stakeholders
Demonstrated ability to lead, motivate, and support team members in delivering quality care
Sound knowledge of relevant legislation, including the Disability Services Act and aged care standards
Current First Aid and CPR certification, or willingness to obtain these qualifications
Valid Working With Children Check and/or National Police Clearance as required
Strong organisational and time management skills with attention to detail
Ability to respond compassionately to challenging situations and resolve conflicts constructively
Physical capability to support activities and mobility assistance as required
Relevant qualifications such as Certificate III or IV in Disability Support, Aged Care, or Community Services are highly valued
