Care Coordinator

We are seeking an exceptional Care Coordinator who has a passion for helping others, a strong commitment to client care and the ability to work collaboratively with a team of healthcare professionals to deliver the best possible outcomes.

Key Responsibilities:

As our Care Coordinator, you will:

• Understand the Commonwealth Home Support program and HACC programs
• Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
• Develop care plans
• Coordinate care
• Provide support and education.

What we're looking for: 

• Current Australian driver's licence
• National police check
• Working with vulnerable people/children check
• Minimum Certificate III in a relevant discipline
